Contact your nearest NECAC Outreach Office to make an appointment to complete the application process.  You will need to bring the following documentation with you to the appointment:

• Documentation from all income sources for the past 6 months
• Photo ID for head of household
• Copies of social security cards for all household members
• Copy of your most recent utility bill for your primary heating source
• If you own your home you will need to bring a copy of the recorded deed to the property
